An in-home practice can only help people inside its service area, and a cash-pay practice can't take Medicare. Braca's Request to Book form asks both up front: the ZIP code where visits would happen, and whether the patient understands the practice doesn't bill insurance.

Conditional logic does the rest. Patients who fit see a short set of questions — what they want help with, how soon they'd like to start, a number to text back — and then the practice's direct line. Anyone outside the area or needing Medicare gets a kind, clear explanation instead of a voicemail that goes nowhere.

Consent to Treatment and a Confidentiality & Privacy acknowledgment go out together as a single packet. Patients see numbered steps with the next one highlighted, finish on their phone, and can come back to it later with their personal code.

After every visit

An invoice for each session

The Visit Invoice & Payment Record is filled in after each appointment: the date, the services provided, what was charged and paid, and how. Repeat entries are on, so the same form logs every visit as its own record — and a patient can take a copy as an itemized receipt for their own insurance, HSA, or FSA claim.

During care

Clinical forms kept in HIPAA mode

Outcome measures like the Neck Disability Index, and incident reports, run in HIPAA mode: responses are stored in TendForm's isolated, encrypted enclave, and every access is logged. New responses notify the team by email — without putting patient details in the message.
